For someone to become a member of Flourish House they must be referred by a Secondary Mental Health Service Practitioner such as a community psychiatric nurse, consultant psychiatrist, psychologist or occupational therapist.

Before filling out an application form, we always recommend that anyone interested in becoming a member book a tour of Flourish House. This gives them the chance to see if we are the right fit! To book a tour call 0141 333 0099.

The form will be filled out by you and the future member before sending it to Flourish House. This must be done by post to ensure confidentiality as our inbox is monitored by both staff and members.
